Primary Responsibilities
🔹 Transitions & Admissions
- Facilitate smooth admissions / discharges for children and families
- Conduct tours, home / hospital visits, and assist with paperwork (Medicaid, Social Security)
- Maintain admissions calendar and complete social histories & annual assessments
- Coordinate care planning and post-discharge support
🔹 Family Liaison & Support
🔹 Interdisciplinary Collaboration
🔹 QIDP Oversight
