Job Title : Licensed Practical Nurse (LPN) – Pediatric School Companion Location : Rochester, IN Position Type : Full-time / Part-time, During School Hours Reports To : Home Health Agency Supervisor / RN Case Manager

Overview : We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide dedicated, one-on-one care for a medically complex pediatric client attending school. The LPN will accompany the client throughout their school day, ensuring continuity of care, safety, and support in both academic and social settings.

Primary Responsibilities :

Provide direct nursing care such as tracheostomy care, ventilator support, tube feedings, medication administration, or seizure monitoring as prescribed.

Monitor the client’s health status throughout the school day, documenting observations and communicating any changes to the supervising RN and caregiver.

Collaborate with school personnel, including teachers and staff, to ensure the child’s safety and successful participation in educational activities.

Assist with mobility, toileting, and activities of daily living (ADLs) as needed.

Respond promptly and professionally to any health emergencies during school hours.

Maintain confidentiality and uphold the dignity of the client at all times.

Qualifications :

Current and valid LPN license in the state of [Insert State]

Current CPR / BLS certification

Pediatric experience, especially in homecare or school settings, preferred

Ability to work independently with confidence and strong clinical judgment

Compassionate, patient, and attentive to both medical and emotional needs

Schedule & Work Environment :

Follows the client’s school schedule, including holidays and breaks

Primarily works in an educational setting with travel to and from school

Must be comfortable in dynamic environments requiring flexibility and discretion
