Weekend Resident Coordinator

We are currently seeking Resident Coordinators who are passionate about making a difference in the community assisting the unhoused population based in our Panorama City and Pacoima locations. Care-givers with experience serving multiple patients, Resident Technicians working previously at a Sober Living Facility and/or Addiction Treatment Center, or individuals who have the drive to helping the underserved are all welcome to apply!

Shifts:
Saturdays: 7:00 am-3:30 pm, 3:00 pm- 11:30 pm,
Sundays: 7:00 am-3:30 pm, 3:00 pm-11:30 pm - Available Floater shifts


Essential Job Functions/ Shift Responsibilities:

• Documents shift summary for each member into our electronic medical record/ EMR system that consists of incidents, observations from visiting disciplines, crises, violations, and emergencies, etc.
• Provide member oversight with medication management.
• Complete temperature and vitals check and complete member log at the beginning of each shift.
• Responsible for admitting new members when they arrive as outlined in the Admission Checklist.
• Maintain a clean and healthy facility for our members; clean the scheduled zone and complete the cleaning log at the beginning of each shift.
• Familiar with any allergies, food, or other allergies members may have and ensure that it is documented and respected.
• Responsible for preparing meals and snacks for all members and must take all dietary restrictions and guidelines for each patient (as provided by their healthcare provider) into consideration when preparing all meals.
• Occasionally support and follow up on transportation that are missing and/or late if the appointment falls within their shift.
• Must have knowledge of where the members are during the shift and must ensure all members log in and out when leaving the facility unless it is for a scheduled appointment on the calendar that the case manager is aware of.
• Provide crisis intervention and possess de-escalation skills
