Job Summary :
The Licensed Practical Nurse (LPN) / Nurse Coordinator is responsible for providing professional nursing care to the agency’s home care clients.
Responsibilities :
- Provide direct client care, data collection, and team collaboration with support to the Registered Nurse.
- Ensure timely and proper completion of all agency related documents and ensure that all documentation meets NYDOH regulations and agency standards.
- Provides supervision of HCSS, PCA, and HHA staff to ensure safety and compliance with plan of care.
- Teach, train, and provide oversight to HCSS, PCA, and HHA staff – including proper use of of durable medical equipment other care aides.
- Complete routine home visits.
- Oversight of the Nursing Provisional Supervision List on a weekly basis to ensure compliance with mandated supervision of aide staff awaiting CHRC clearance.
- Follow up for all clients hospitalized and in rehab to act as point of contact for tracking of when clients are returning home.
- Tracking and obtaining of signed MD orders and interim orders along with the Manager if Information Systems to ensure compliance with obtaining signature of MD orders.
- Scheduling of nursing visits in AlayaCare to ensure that billable visits are captured, and support centralized scheduling and appropriate utilization of client staff.
- Audit support under the direction of the Director of Clinical Services.
- Classroom support under the direct supervision of an RN.
- Prefill medication for clients per medical order and Plan of Care.
- Perform agency requested “Meet & Greets” to ensure appropriateness and ability to provide a safe plan of care in the home environment.
- Perform client’s personal care and / or assist with activities of daily living as needed.
- On-call and after office hours may be required at times.
- Perform other duties as assigned.

Director of Clinical ServicesPay : $22 - $25 per hour
Position Requirements :
Graduate of an accredited School of NursingValid and current New York State Licensed Practical Nurse license and registration.Previous home care experience preferred.Ability to successfully complete a Competency & Proficiency Test and Skills Evaluation appropriate to job title before hire.Excellent verbal and written communication skills.Excellent human relations and customer service skills.Ability to function with minimum direct supervision, to make decisions, to problem solve, and to manage emergencies.Ability to relate to diverse and multi-cultural populations.Appearance is professional and complies with agency dress code.Completion of a background check.Ability to lift up to 25 pounds and to sit, stand, bend, stoop, kneel, pull, climb, reach, and perform repetitive movements of the upper extremities.Pass a physical examination and immunization requirements after being offered employment and complete a health assessment annually, thereafter.Possess a current, valid New York State driver’s license and a reliable vehicle.Completion of CCOR's orientation program.Environmental Conditions :
