DEPARTMENT SUMMARY : The Senior Services department supports and enriches the lives of lesbian, gay, bisexual and transgender (LGBT) people 50 years and older, with an overarching goal of successful aging in place. In this effort, the department provides a broad array of social, educational and support services to LGBT seniors, all of which are free or low-cost. We have an expanding population of LGBT homeless population and are growing as a department to include on site affordable housing for LGBT seniors.

JOB SUMMARY : Under the supervision of the Sr. Manager of Social Services and as part of the Seniors Services team, the Senior Care Specialist I will develop individual care plans for LGBT seniors (50+) throughout Los Angeles County. The ideal candidate possesses the capacity to work with LGBT Seniors addressing the unique needs of the population and ensuring an optimal level of continuum of care often collaborating with external agencies and LGBT-friendly service providers. The Senior Care Specialist addresses a wide range of subjects that pertain to their client's well-being utilizing interventions which include : comprehensive assessment, care planning, service authorization or arrangement, case monitoring and client advocacy. Service provision may be delivered at any of our locations including, but not limited to : Anita May Rosenstein Campus, Ariadne Getty Foundation Senior Housing, Triangle Square, Metropolitan Community Church, Mi Centro, and other offsite locations.

ESSENTIAL FUNCTIONS :

• Conducts in-depth intakes and assessments covering psychological, social, physical, and environmental concerns for LGBTQ Seniors within Los Angeles County.
• Assesses the client's functional and cognitive capacity and limitations.
• Develops a comprehensive care plan including all social services needed for client to meet their immediate or urgent needs, as well as longer-term needs.
• Provides support and guidance addressing barriers to obtaining services such as housing and appropriate healthcare needs.
• Implements the care plan by coordinating and arranging for services.
• Assists clients with linkage to social service resources for seniors including, but not limited to : Medi-Cal / Medi-Care, CalFresh, affordable housing, mental health, healthcare, legal and Veteran Affairs benefits, In Home Supportive Services and home delivered meals.
• Routinely follows up and monitors clients in care to ensure that the care plan continues to meet the clients' need (due at the beginning of each month).
• Develops and incorporates housing stabilization into any care plan created for residents of Triangle Square and AGF Senior Housing which includes in-home care provisions, higher levels of care and / or end of life care based on assessments.
• Ensure housing stabilization and retention are in partnership with property management lease agreements and tenant plans to ensure housing stability.
• Completes comprehensive reassessment on a minimum of an annual basis or more frequently given the client's needs.
• Follows termination of care management services protocols when appropriate.
• Identifies and develops family and other informal support systems for the client.
• Ensures appropriate documentation is entered into CIVI and other required data systems to meet all Department of Aging requirements.
• Maintains client-related data tracking systems, including case notes, complete CIVI entries and CTS data entry
• Prepares case-related reports including, but not limited to : outcomes, successes and challenges on a monthly basis (report template provided).
• Maintains complete and accurate documentation of service objectives and outcomes as well as other services;
• Provides clients with appropriate internal and external referrals to address identified needs and serve as an advocate for LGBT seniors with outside agencies.
• Develops and maintains a comprehensive resource and referral database of LGBT specific resources and community resources relevant to participant needs.
• Offers clients the opportunity for regular feedback on services provided to measure effectiveness of services and interventions utilized.
• Participate in care management meetings, all-staff meetings, in-service trainings, case consultations and other meetings, as assigned and in coordination of care support.
• Other duties as assigned.
