As Supervisor ElaraConnect Engage Program, Non-Clinical, you'll contribute to our success in the following ways :
Maintains patient and staff privacy and confidentiality in compliance with HIPAA requirements.Supports effective and efficient systems for patient referrals, ensuring proper intake, eligibility verification, and communication of referral information.Oversees daily, weekly, and monthly reporting processes to ensure timely submission to leadership.Provides direction and oversight to non-clinical personnel to ensure delivery of quality services.Supervises staff to support coordination of patient care activities.Maintains regular communication with department leaders and direct reports to support workflow and collaboration.Builds relationships within the company by maintaining visibility and accountability.Participates in assigned special projects.Connects customers with appropriate internal partners based on identified needs (e.g., skilled care, personal care, alert follow-up, healthcare coordination).Maintains organized systems and effective communication processes to support continuity of quality patient care.Assists in evaluating operations and identifying opportunities to improve efficiency and productivity.Performs other duties / projects as assigned.What is Required?
High School Diploma / GED is required.2+ years experience within Home Health is required.Experience in Homecare Homebase EMR is preferred.Demonstrates strong verbal and written communication skills.Proficiency in MS Office Suite and data entry skills.Must have current drivers license, reliable transportation and insurance in accordance with the laws of the state.Must be able to sit, stand, bend, lift and move intermittently and be able to lift at least 25lbs.We value the unique skills of veterans and military spouses.
