Overview\nWe are seeking a dedicated and compassionate Health Home Registered Nurse Case Manager to join our team. - Conduct thorough patient assessments to develop individualized care plans.\n
- Coordinate and manage patient care across multiple disciplines, ensuring continuity of care.\n
- Collaborate with healthcare providers to facilitate discharge planning and follow-up care.\n
- Monitor patient progress and adjust care plans as needed based on clinical evaluations.\n
- Provide education to patients and families regarding health conditions, treatment options, and self-management strategies.\n
- Maintain accurate medical records in compliance with HIPAA regulations.\n
- Utilize critical thinking skills to evaluate diagnostic information and make informed decisions regarding patient care.\n
- Engage in utilization management activities to optimize resource use while ensuring quality patient outcomes.\n
- Support patients with complex needs including geriatrics, behavioral health, and infectious disease care.\n
- Participate in case management meetings to discuss patient status and share insights with the healthcare team.\n\nExperience\nCandidates should possess the following qualifications : \n
- Critical care experience in settings such as ICU, emergency room, or Level I / II trauma centers.\n
- Familiarity with various medical specialties including pediatrics, geriatrics, hospital medicine, outpatient clinics, and medical-surgical nursing.\n
- Knowledge of medical terminology, physiology, and patient service best practices.\n
- Experience with discharge planning, intake processes, and case management principles.\n
- Strong understanding of acute care protocols and post-acute care transitions.\n
- Proficiency in conducting diagnostic evaluations and managing medical records effectively.
