Responsibility Summary :

• Performs vital signs and activities of daily living as outlined on plan of care inclusive of ADL's.
• Assists on hourly rounding and documents care rendered per guidelines.
• Meets nutrition needs of patients as outlined in the plan of care, and in accordance with policies and procedures.
• Properly documents per policy, protocol, and procedure.
• Maintains patient comfort and safety as outlined in the plan of care and in accordance with policies and procedures.
• Assures rehabilitation needs are met as outlined in the care plan and in accordance with policy and procedures, including ROM, ambulation, transfers, proper body alignment, and use of assistive devices.
• Appropriately participates in shift changeover communication providing complete and relevant information about patients.

Experience : Minimum 1-2 years customer service and / or health related experience.

Education : Minimum : High School graduate or equivalent education.

Salary Range : $19.09-$21.72
