RN Case Manager – Home Health – South Fulton

Visiting Nurse Health System is seeking an RN Case Manager for Home Health who will play a critical role in delivering comprehensive nursing care to patients in their homes, ensuring continuity and quality of care outside traditional clinical settings. This position involves assessing patient needs, developing individualized care plans, and coordinating with multidisciplinary teams to optimize patient outcomes. The RN Case Manager acts as a liaison between patients, families, healthcare providers, and community resources to facilitate effective communication and support. By monitoring patient progress and adjusting care plans as necessary, the role ensures adherence to clinical guidelines and regulatory requirements. Ultimately, this position aims to enhance patient independence, reduce hospital readmissions, and improve overall health and well-being in the South Fulton County community. In this position…

  • Conduct thorough patient assessments in home settings to evaluate health status and care needs.
  • Accurately and timely documents using HCHB and submits OASIS documents
  • Develop, implement, and regularly update individualized care plans tailored to each patient's condition and goals.
  • Coordinate with physicians, therapists, social workers, and other healthcare professionals to ensure integrated care delivery.
  • Monitor patient progress, document clinical findings, and adjust interventions to promote optimal health outcomes.
  • Educate patients and their families on disease management, medication adherence, and preventive health practices.
  • Facilitate communication among all parties involved in patient care to ensure clarity and continuity.
  • Ensure compliance with all regulatory standards, documentation requirements, and organizational policies.

Do you have…

  • Bachelor of Science in Nursing from an accredited School of Nursing, or an Associate Degree in Nursing.
  • Current and valid Registered Nurse (RN) license in the state of Georgia.
  • Minimum of two years of clinical nursing experience, preferably in home health or community health settings.
  • Basic Life Support (BLS) certification.
  • Strong knowledge of nursing principles, patient assessment, and care planning.
  • Ability to work independently and manage a caseload effectively.
  • Valid GA driver’s license, proof of auto insurance, and reliable transportation
