Licensed Clinical Adherence Coach

A company is looking for a Clinical Adherence Coach (RN / LPN).

Key Responsibilities :

Conduct outreach via phone, text, and video to educate and enroll patients into the adherence program

Provide compassionate coaching, case management, and health education services to encourage medication adherence

Assess patient needs and escalate cases as necessary while maintaining accurate documentation of interactions

Required Qualifications :

Licensed Practical Nurse (LPN) license in good standing

3-5 years of related experience in patient engagement, health education, or care coordination

Strong background in chronic disease management and medication adherence

Proficiency in electronic health records (EHR) and digital health platforms

Experience assessing social determinants of health and connecting patients with resources
