LOCATION : Venice, CA and field-based

ABOUT THE ROLE

The Community Connection Case Manager position will support the flow and responsibilities of the Clinical Department and Access Center Programming, and will work closely with a caseload of members towards a variety of youth centered goals. This role is an integral part of case management and clinical services at SPY with both field- and Access Center-based responsibilities.

WHAT YOU’LL DO :

• Support the SPY Clinical and Access Center Teams in day to day operations and programming.
• Utilize innovative outreach strategies to enroll young people into SPY’s case management programs.
• Support members in building and deepening social connections through planning and facilitation of community field trips and onsite gatherings with transitional age youth.
• Conduct intakes, assess needs, and create youth centered goal plans to foster youth development, social connection, housing stability, and wellness.
• Complete referrals and support youth with resource navigation to promote successful connection to needed services and programs in alignment with their goals plans, including but not limited to housing, mental health, parenting resources, benefits, social support, medical care, education, and employment.
• Support youth with reunification, reconnection, and relocation as needed.
• Contribute to Access Center Front Line operations, ensuring an inclusive space where members are welcomed and connected to SPY program offerings.
• Use a harm reduction, trauma-informed, housing-first model when working with youth.
• Provide mediation, crisis intervention, and restorative justice member accountability processes with youth, as needed.
• Participate in a multi-disciplinary team approach to support youth driven care plans.
• Complete documentation, activity tracking, surveys, and reporting in a timely fashion in HMIS / Clarity, SPY electronic health record, and other data tracking systems.
• Build relationships within the network of community providers to enable appropriate referrals.
• Advocate for and with youth by attending external community meetings with government agencies and community groups.
• Work collaboratively across internal departments and assist with general SPY programming as needed.
• Work within established legal and ethical guidelines.
• Other duties as assigned to support the mission of the department and organization.

Mandated Reporting Requirements : The person holding this position is considered a ‘mandated reporter’ under the California Child Abuse and Neglect Reporting Act and is required to comply with the requirements set forth in CSU Executive Order 1083 Revised July 21, 2017 as a condition of employment.

COVID-19 Vaccination Requirements : Effective August 9, 2021, the State Health Officer has mandated that all workers, paid or unpaid, in homeless services organizations must provide proof of vaccination. If eligible for a booster shot, proof of booster is also required. If not fully vaccinated, weekly COVID-19 testing is necessary depending on the program.
