About The Position

Job Title : Physical Therapy Assistant

Job Specialty : Rehabilitation Therapy - Physical Therapy

Job Duration : 13 weeks

Shift : Monday-Friday, Days

Guaranteed Hours : 32 hours per week

Experience : New grads are welcome; Skilled Nursing Facility (SNF) experience preferred

License : State License

Certifications : CPR Certification

Must-Have : Completed prescreen form, attached resume, and license verification

Job Description :

• Ensure prescreen form is filled out and attached upon submitting the application; incomplete submissions will be declined.
• Conduct COVID-19 testing 72 hours prior to the start date, with results available 48 hours before starting.
• Complete credentialing checklist prior to submission, which includes :
• Resume and three references
• County Criminal background check (7 years for all counties lived and worked)
• Federal Criminal background check (7 years or more)
• Multi-State Criminal Search (7 years or more)
• Nationwide Sex Offender Search (7 years or more)
• HHS / OIG Search
• GSA Search
• Social Security Number Trace
• 1 Step PPD or Quantiferon or Proof of Positive PPD and Chest X-Ray
• SAM Search
• CPR certification (fundamental facilities)
• Negative drug screen results
• Familiarity with rehabilitation therapy processes and patient care techniques.
• Ability to work collaboratively with a team of healthcare professionals.
• Strong communication skills with patients and other staff members.
