Job Description
The Occupational Therapist assesses and treats patients in the home in order to assists them in the development and execution of activities of daily living.
- Evaluates patient's functional status as well as the home environment to identify hazards or barriers to achieve more independent living
- Performs appropriate diagnostic musculoskeletal, neurological and functional skills tests to develop a treatment plan that establishes goals for improved function
- Communicates plan of care to referring physician and collaborates with other clinicians to ensure patient needs are met
- Educates patients on the use of various types of adaptive equipment to improve function
- Provides instruction on proper posture / body mechanics to promote prevention of injury and maximize therapeutic outcomes
- Educates patients concerning how to maximize quality of life within the limits of their physical capabilities
- Provides guidance and education to family members and caregivers to ensure patients continue to sustain improvement with their physical, mental or cognitive disorders
- Documents all changes in patient's condition and updates care plans and outcomes accordingly
- Supervises the care provided by Certified Occupational Therapy Assistants to ensure proper care is delivered
- Instructs, supervises, and evaluates home health aide care every two (2) weeks when therapy is the only skilled service
- Participates in discharge planning for the patient
Requirements
Basic Qualifications
Must be a graduate of an accredited school of occupational therapy and holder of a current permanent or temporary license from the Indiana Medical Licensing BoardAbility to demonstrate knowledge and technical skill necessary for providing competent, comprehensive patient care.CPR certification required.Must possess an Indiana Driver's license and automobile liability coverage as required by the State of Indiana.Ability to travel to all regions serviced by the agency in various weather conditionsMust have the ability to perform essential job functions with or without reasonable accommodation
