We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our newly developed Memory Care Unit .

Position Details :

Licensed Practical Nurse (LPN) - Memory Care Unit

Schedule : Saturday - Sunday, Days, 8 hour shifts

Supportive environment where residents and staff are valued

Key Responsibilities :

• Administer medications and treatments in accordance with care plans
• Oversee daily care provided by CMTs and CNAs
• Assist with admissions, discharges, and care transitions
• Communicate effectively with residents, families, and interdisciplinary teams
• Monitor and report changes in resident health status

Job Requirements

Applicable Experience :

1 year

Basic Life Support - American Heart Association or Red Cross, Licensed Practical Nurse - Various
