As a Case Manager at Pinnacle Services, you will be instrumental in enhancing the lives of our clients through meticulous planning, coordination, and advocacy. Assess client needs through comprehensive interviews and assessments. Develop, implement, and review individualized care plans in collaboration with clients, families, and other healthcare professionals. Facilitate access to healthcare services, social services, and community resources tailored to each client's needs. Monitor and report on the progress of clients, adjusting care plans as necessary. Maintain accurate records and documentation in accordance with legal requirements and organizational policies. Advocate for clients? rights and best interests during interactions with healthcare providers and other agencies. Provide crisis intervention as needed and coordinate with other professionals to ensure a comprehensive support system.
