JOB SUMMARY :

CHSI is looking for a passionate primary care physician who is interested in providing full-spectrum primary care to vulnerable and medically and socially complex patient population.

DUTIES :

Provides primary health care to patients in Community Health Service Inc. practice sites

Performs outpatient procedures within the scope of primary care.

May assist the Medical Director in revision and implementation of practice principles, clinical protocols and health care plans as needed.

Follows all applicable national patient safety goals in the provision of patient care.

Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude. Leads the team in the team-based care model

Provides care in collaboration with the center's team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.

Serves as a resource person for clinical staff regarding patient care issues.

Establishes and maintains rapport with community agencies.

Performs duties essential to the position and program.

Demonstrates a sincere dedication and loyalty to the philosophy and mission of CHSI

PHYSICAL REQUIREMENTS : (Initial by duties you are able to perform)

In an eight hour workday, the employee must be able to : bend and crouch occasionally,

sit continuously and stand or walk frequently.

  • Employee must be able to lift up to 50 pounds occasionally.
  • Employee must be able to use hands / arms, feet / legs for repetitive fine to large motor skill

movements.

REQUIRED QUALIFICATIONS :

MD / DO with board certification in Family Medicine, Internal Medicine or Pediatrics. Strong preference for a physician with women's health background.

Valid state medical license / DEA in jurisdiction of clinic required.

Basic Life Support (BLS) required.

Ability to diagnose and treat conditions within the scope of full-spectrum primary care.

Knowledge and understanding of the principles and standards of clinical practice in outpatient medicine.

Thorough knowledge of health promotion and disease prevention strategies.

Ability to work as a team member, establishing and maintaining effective relationships with patients and other staff.

DESIRABLE QUALIFICATIONS :

Experience in working in community health center, public health or in a similar type of program.

Ability to communicate effectively in English. Bilingual provider preferred (Common languages served : Spanish, Somali, Arabic, Amharic)

Knowledge and understanding of socio-economic and health problems of underserved populations is advantageous.
