Job Summary
We are seeking a dedicated and compassionate Qualified Professional to support and advocate for Service Recipients receiving Innovations Waiver or State Funded Services. In this vital role, you will help individuals lead meaningful lives by ensuring they receive high-quality care and personalized support.
Key Responsibilities
Develop individualized short-term goals and interventions that create positive, measurable outcomes.
Conduct clinical and employee supervisory visits to ensure quality care and compliance with standards.
Create and oversee development plans and annual performance evaluations for direct care staff.
Act as an advocate during Individual Support Plan (ISP) meetings, ensuring recipient needs and preferences are represented.
Monitor billing accuracy, maintain thorough documentation, and address grievances efficiently and empathetically.
Participate in community events to promote awareness and strengthen connections within the community.
Qualifications
Bachelors degree in a related human service field with 2 years of full-time IDD experience, OR
Bachelors degree in an unrelated field with 4 years of full-time IDD experience, OR
Masters degree in a related human service field with 1 year of full-time IDD experience.
