HIM CLERK POSITION DESCRIPTION JOB TITLE : HIM CLERK II Supervised By : HIM Supervisor Prepared by : _______________________ Approved By : __________________ Date : ______________________________ Date : __________________________ JOB SUMMARY : Review outpatient medical records post-discharge for completeness in compliance with the hospital’s bylaws and state requirements. Coordinate record reviews and aid with gathering other information when needed. Respond to requests for medical release of information by performing duties per the established hospital and departmental policies and state and federal laws. Scans all health data into the EHR (electronic health record) DUTIES AND RESPONSIBILITIES :
- Always maintain patient confidentiality.
- Process outpatient discharged charts from the patient care units daily.
- Analyzes medical records of discharged patients for completeness and accuracy according to department and hospital policy and procedures.
- Flag charts for deficiencies and keep a deficiency log.
- Analyze records to ensure proper order before scanning.
- Scan records into the appropriate areas of the EHR
- Sort records appropriately within the EHR
- Responsible for coordinating the release of medical information to patients, physicians, and other health information entities
- Verifies authorizations per hospital policy and procedure and state and federal laws.
- Maintains a release of information log.
- Maintains hospital requirements, policies, and standards on confidentiality.
- Maintains a good working relationship within the department, other departments, and the medical staff.
- Willingness to accept more assignments.
- Answers phone calls professionally and courteously , takes messages, and routes calls to the proper staff members.
- Performs clerical work as needed (fax, copies, and mail)
- Adheres to the dress code – the appearance is neat and clean.
- Completes annual education requirements.
- Maintains regulatory requirements.
- Reports to work on time and as scheduled.
- Completes work within the designated time.
- Wears identification while on duty and uses the computerized punch time system correctly.
- Actively takes part in performance improvement and CQI activities.
- Follows organizational policies per ethical business practices.
- Communicates the facility's mission, ethics, and goals and the department's focus statement.
Requirements :
High school graduate or equivalent.
Two (2) or more years of prior hospital or clinical experienceKnowledge of medical terminologyAbility to work with physicians collaboratively.Ability to read and communicate effectivelyBasic computer knowledge.PHYSICAL DEMANDS : Includes vision, hearing, and repetitive motion. Must sit for extended
periods.
