Job descriptionCoordinates patients through the acute care hospitalization Manages all aspects of discharge planning / transition process. Performs utilization management and quality screenings. In- Patient Case Management is required.
Job Title : Case Manager
Location : Morristown, NJ
Duration : 3+ month contract (Possibilities of Extension)
Shift : 8 am – 4 pm Monday - Friday (5
- 8) hrs shift, may require weekends coverage.
Stipend is provided to the candidate living more than 50 miles from the facility.
Payrate : $57 / hour. on W2
