Required Qualifications : Graduation from an accredited Licensed Practical Nurse / Licensed Vocational Nurse (LPN / LVN) school of nursing or Physical Therapy Assistant (PTA) program. Within 30 days of hire : National Provider BLS - American Heart Association Upon hire : Oregon Licensed Practical Nurse Upon hire : Washington Licensed Practical Nurse - if practicing within this state. 2 years of experience as an LPN / LVN (Acute, Clinic, or Long Term Care). Preferred Qualifications : 1 year of Home Health experience. 1 year of Triage experience. Why Join Providence?
