The Targeted Case Manager – D&A Specialist will provide services to participants, living in the community with persistent mental health and drug and alcohol challenges. After assessing needs, the Drug and Alcohol Specialist helps the participant with obtaining resources and services in the community, in a partnership that focuses on recovery and the achievement of the participant’s goals for community living. The Drug and Alcohol Specialist also is instrumental in assisting homeless individuals to locate adequate shelter / residence. The Drug and Alcohol Specialist also engages individuals who are incarcerated acting as a community link in reestablishing a productive life within the community once released.

The D&A Case Manager is a full time position, Monday - Friday, 8 : 30 AM - 5 : 00 PM ( with the opportunity to create a flexible schedule with manager's approval ) and is located at 414 East High Street, Pottstown, PA 19464 . This is a hybrid work from home position, with required in-person participant visits and office days and ability to work from home 1 day a week. The rate of pay for this position will be an exact rate of $24.00 / hour .
