JOB SUMMARY : To provide the best possible personal care to ill and disabled persons in the home for the achievement of continuity in basic health care. The Home Health Aide functions under the directions and supervision of the CHHA Coordinator.
QUALIFICATIONS :
- High school diploma or GED. Must be able to read and write.
- Must be proven competent in accordance with OBRA.1987.
- Preferable completed one year of employment as a Nursing Assistant or Home Health Aide.
- Must be in good health.
- Must complete the nurse aide skills assessment test with a score of 75% or better.
- Must demonstrate competency in the personal care tasks as evaluated by a Registered Nurse using the agency checklist for HHA skills.
PERFORMANCE RESPONSIBILITIES :
Provide direct personal care as set forth in the Home Health Aide care plan, including but not limited to :Personal hygiene; bath, shampoo and hair care, oral hygiene.Application of lotion for skin lubrication.Back massage, positioning of bed-bound patient and passive exercises.Assisting patient, transfers to or from bed or chair.Maintaining a healthful environment including cleanliness of the patient's immediate environment.Reports significant data regarding the patient's condition or factors influencing the patient's welfare to the CHHA coordinator.Completes the Home Health Aide clinical notes on every visit and submits them to the CHHA coordinator in a timely manner.A professional Registered Norse will make follow-up supervisory visits at least every two (2) weeks.Will participate QA activities as assigned.Will attend case conference meetings, in-services and staff meetings.484.36c Standard : Assignment and duties of the Home Health Aide.The home health aide is assigned to a particular patient by the CHHA Coordinator. Written instructions for patient care are prepared by a Registered Nurse or Therapist as appropriate.Duties include the performance of simple procedures as an extension of therapy services, personal care, ambulation and exercise, household services essential the health care at home, assistance with medications that are ordinarily self-administered.Reporting changes in the patient's conditions and needs.Completing appropriate records.EVALUATION : Initial 90 day and Annual appraisal by CHHA Coordinator.
PHYSICAL LABOR REQUIREMENTS : Functioning auditory, speech and visual skills. Must be capable of excessive standing, bending and heavy lifting. Must be self-mobile.
DEGREE OF TRAVEL : Daily home visits, weekly office meetings. Requires dependable transportation.
DEGREE OF DISRUPTION OF ROUTINE, OVERTIME : Must be able to adapt to client's status and needs, must be self directing with ability to work with little direct supervision, should demonstrate patience, tact, good personal hygiene and emotional health.
Any employee who discloses confidential business information will be subject to disciplinary action including possible discharge and legal action, even if he or she does not actually benefit from the disclosed information.
Employees who work overtime without receiving prior authorization from the supervisor may be subject to disciplinary action up to and including possible discharge.
