• Shift : 12 hours, days

• Experience :
• 2 years of experience required
• Must have experience with inpatient Hospice
• Must have HCHB / OASIS experience
• Oasis E - after Jan 2023 experience is required
• No more than 1 year gap
• All documentation due same day
• Homecare Homebase (Productivity points, minimum average26-28)
• Absolute must - true home health experience, HCHB / Oasis documentation
