Nurse Supervisor RN

Yanet’s Family Home Care is seeking a full-time Nurse Supervisor. This position will require a highly organized, driven, dynamic, detail-oriented, flexible leader to maintain a full case load. Perfect for an RN seeking a rewarding career in home health care, interested in building strong relationships with clients at a boutique home health care agency, and making a difference in the lives of elderly and disabled patients in our local community.

Our Nurse Supervisor is responsible for maintaining and elevating our standards of nursing care and implementing the policies and procedures established by the agency. Our RN will also be responsible for creating and implementing a medical plan of care that is prescribed by a licensed physician. We need someone with excellent communications skills in Spanish and English (a must!) and the ability to reliably travel to patient’s homes to open new cases and perform assessments to ensure our Certified Home Health Aides are providing exceptional personalized care to our valued clients.

Primary Responsibilities

• Competently assess and document the client’s biophysical, psychosocial, and educational needs
• Interprets to the client and family the implications of the client’s health status
• Consistently report appropriate changes in the client’s condition to the Director Of Nursing and the physician
• Administers and documents the client’s prescribed medication competently
• Oversee and assess caregiver’s adherence to the prescribed plan of care
• Deliver care within the guidelines of ANA standard and the policies set forth by Yanet’s Family Home Care

Required Knowledge/Skills/Licenses

• Graduate of an accredited School of Nursing required
• RN license in good standing, holding a current/appropriate license/registration in the state of New Jersey. A multi-state license is acceptable.
• Bilingual fluency in Spanish and English (absolutely required)
• One year of professional experience as a Registered Nurse
• Experience in home care nursing preferred
• Availability via telephone and to provide home visits, if necessary, during Agency On-Call Hours
• A valid driver’s license
• A valid auto insurance policy with liability coverage
