We are seeking a full-time Nurse Practitioner to provide comprehensive medical management within long-term care and skilled nursing facilities in the Baton Rouge region.

Responsibilities
- Provide primary and post-acute medical care in skilled nursing and long-term care facilities
- Perform admission, regulatory, routine, and acute visits
- Conduct comprehensive physical exams and manage chronic and acute conditions
- Monthly order reviews and gradual dose reductions for psychotropic medications.
- Order and interpret diagnostic tests, labs, and imaging
- Prescribe and manage medications and individualized care plans
- Collaborate with interdisciplinary facility teams to support quality outcomes
- Ensure compliance with federal and Louisiana long-term care regulations
- Identify changes in condition and intervene to prevent avoidable hospitalizations
- Support goals-of-care discussions and advance care planning
- Participate in after-hours or on-call rotation as needed
Qualifications
- Active Nurse Practitioner license in Louisiana
- 1 year experience as a Nurse Practitioner preferred
- DEA license required
- Experience in long-term care, skilled nursing, hospice, or post-acute care preferred
- Strong critical thinking and clinical decision-making skills
- Excellent time management and ability to work in a fast-paced environment
- Ability to work independently while collaborating with facility staff
- Strong documentation and communication skills
Preferred
- Familiarity with Louisiana long-term care regulations
- Experience in SNF/LTC medical management
- Experience with PCC or other long-term care EMRs
- Background in value-based or high-risk population management
