Job Overview
Were seeking a dedicated Behavioral Health Clinician to join the MGB Diabetes Initiative, providing integrated behavioral health support to patients with or at high risk for cardiometabolic conditions such as diabetes, hypertension, and obesity. In this role, you'll work collaboratively with a multidisciplinary team to address behavioral health needs, support patient self-management goals, and advance health equity. The ideal candidate is compassionate, mission-oriented, skilled in both individual and group behavioral health care, and thrives in community-facing work.
Responsibilities / Clinical Duties
Assess and address behavioral health needs for patients with cardiometabolic conditions
Respond to referrals and proactively engage high-risk patients
Facilitate patient-centered care plans for chronic condition management
Participate in community programs and events (occasionally off-hours)
Provide care both virtually and in-person (hybrid schedule)
Conduct psychosocial assessments, crisis intervention, diagnosis, and short-term treatment plans
Adhere to trauma-informed, evidence-based care practices
Present cases in team meetings and communicate recommendations
Monitor patient outcomes using validated tools (e.g. PHQ-9)
Facilitate group sessions and behavioral health education
Lead behavioral health group visits & provide individual therapy
Other Responsibilities
Attend IBH / Psychiatry meetings and training opportunities
Participate in program huddles and collaborative planning
Partner with community teams to develop nutrition and economic mobility programming
Maintain licensure and continuing education
Follow confidentiality, safety, infection control, and documentation standards
