Job Summary : We are seeking a highly skilled and compassionate LPN to join our dynamic healthcare team by the next quarter. The ideal candidate will be responsible for coordinating and managing patient care remotely, utilizing telehealth platforms to provide efficient and effective healthcare services.

Requirements :

• Licensed Practical Nurse (LPN) with active license
• 3-5 years of current experience helping patients manage chronic conditions (Required)
• 2-5 years of care management experience with SDoH and behavioral health
• Current experience and knowledge of patient education methods (Required)
• Must have effective telephone interaction skills to engage patients successfully
• EMR and Patient Management Platform use and experience (Required)
