Telephonic Case Manager RN

A company is looking for a Telephonic Case Manager RN Transplant Medicine - Remote.

Key Responsibilities

Make outbound and receive inbound calls to assess members' current health status

Identify gaps or barriers in treatment plans and provide patient education for self-management

Coordinate care and make referrals to outside resources while ensuring a complete continuum of quality care

Required Qualifications

Current, unrestricted Compact RN license in the state of residence

3+ years of experience in a hospital, acute care, or direct care setting

Proficiency in Microsoft Office Tools and Systems (Outlook, Word, Excel, Teams)

Access to high-speed internet (DSL or Cable) and a dedicated work area for privacy
