Deliver exceptional, integrated care that profoundly impacts the lives of our diverse clientele, from the youngest infants to the most experienced adults.Enhance your skills in patient counseling, drug utilization reviews, and medication adherence, making a significant contribution to our healthcare ecosystem.Immerse yourself in a culture that values collaboration, mentorship, and personal growth.Be a crucial part of a mission-driven organization dedicated to improving community health.Key Responsibilities Perform essential dispensing tasks, including data entry, medication preparation, and sales completion while providing insightful patient counseling.Conduct outreach and enroll select patients in our specialty pharmacy program, supporting ongoing pharmacy initiatives with proactive communication.Ensure the accuracy and appropriateness of medication orders by reviewing patient profiles, provider notes, and relevant laboratory information.Maintain compliance with all regulatory authorities, upholding rigorous standards of pharmacy operations.Engage in comprehensive Drug Utilization Reviews to detect drug duplications, interactions, or allergies.Document and communicate adverse drug reactions and patient interactions to healthcare providers.Provide educational guidance to patients, families, and healthcare providers about drug interactions, side effects, and proper pharmaceutical storage.Assess and promote medication adherence through targeted counseling sessions.Administer immunizations following established protocols.Exhibit a cooperative spirit and set priorities effectively while working both independently and collaboratively.Convey information clearly and concisely, fostering positive interactions with all healthcare professionals.Approach interactions with friendliness, compassion, and sensitivity, ensuring a supportive healthcare environment.Minimum Qualifications Active enrollment in an accredited college or university of pharmacy.Eligible or certified in immunization delivery, smoking cessation, and naloxone administration.Possess a valid Pharmacist Intern License from the Texas State Board of Pharmacy.Maintain current Basic Life Support (BLS) certification, including CPR.Experience or familiarity with specialized fields such as HIV / AIDS, Geriatrics, and / or Pediatrics is preferred.About Legacy Community Health As the largest Federally Qualified Health Center (FQHC) in Southeast Texas, Legacy Community Health has been delivering comprehensive, high-quality, and affordable health care for nearly 40 years.With more than 50 clinics across the Greater Houston and Gulf Coast region, we continue to grow strategically and invest in our communities.By innovating how care is delivered, we've stayed at the forefront of connecting our communities to health—every day, in every way.At Legacy, we know our success is powered by our people.We're always looking for talented, passionate individuals who want more than a job—they want a meaningful career that makes a real impact.Explore our open positions and see if a career at Legacy is right for you.At Legacy Community Health, our mission—Driving healthy change in our communities—guides everything we do.To fulfill this mission, every team member is expected to embody the following core attributes in their daily work, regardless of role or department :
