Primary Job Responsibilities : Assess a person s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a person s needs are met. Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests. Utilize a variety of therapeutic modalities to maximize care provided to clients Requirements : Vermont PT licensure Home Health experience and OASIS experience a must as this candidate will case manage McKesson / Netsmart EMR experience strongly preferred Position Billing string = Central Vermont Home Health & Hospice Guaranteed Hours : 36 Contract
