1.General job duties. Types of procedures required.

Evaluation, reassessment, and treatment for physical therapy conditions. Works closely with team including additional PT and PTA, along with OT at this location. Inpatient acute referrals and swing bed patients are evaluated and treated to meet medical need. Outpatient clinic is rural-based healthcare, and variety of conditions. Traveler would not be expected to provide specialized services such as pelvic floor, but specialties are welcomed.

2. Equipment utilized in the unit / dept. (Is there computerized charting, medication systems, etc.)

EPIC documentation, rehab equipment, computerized HEP program (MedBridge)

3.Required skills and minimum years of experience.

Applicant must have a PT license in good standing from the state of Illinois. Position includes inpatient and outpatient services, so experience in both is needed. Recommend at least 1 year of experience at minimum

4.Specific licensures and certifications / registrations required. (ARRT, CRT, BLS, ACLS, etc)

CPR, PT license Illinois, Valid Drivers License

5. Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)

Monday through Friday 830-5 (could be 8-430 if needed)

6. Bed size / number of rooms for this unit / dept.

8-10 inpatient beds

7.Dress Code : Business Casual

Rehab has a defined orientation process including competencies, review of policy and procedures, EPIC training, and patient care processes
