Position : Primary Care Physician

Pay : & $260,000.00 - $312,000.00 per year

Location : Jacksonville, Florida, Pennsylvania, North Carolina, Arizona, Ohio, Wisconsin, Missouri, Arkansas

Position & Type : & Full-Time

Shift : & Monday-Friday 8 : 00 AM – 5 : 00 PM

Compensation :

  • Base Salary : $260,000
  • Bonus : Up to 20% of base salary based on :
  • Patient satisfaction ratings
  • Alignment with organizational values
  • HEDIS compliance metrics
  • Reduction of hospital sick days
  • Model : Value-Based Care (Outpatient)
  • Patient Population : Seniors with 3–5 chronic conditions (e.g., heart disease, diabetes, hypertension)
  • Patient Load : 10–12 patients per day
  • Panel Size : Maximum of 650
  • Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM
  • Call Schedule : Based on market provider count (e.g., 1 : 32 weeks if 32 physicians in region)
  • Care Team Support : Each physician is partnered with a Medical Assistant, Scheduler, Referral Specialist, and full administrative and leadership support

    • Requirements :

  • Doctor of Medicine (MD) or Doctor of Osteopathy (DO)
  • Board Certification in Family Medicine or Internal Medicine (Required)
  • Fellowship in Geriatrics (Preferred but not required)
  • Current, unrestricted medical license in the applicable state
  • Current DEA registration
  • Strong proficiency in office-based procedures, including :
  • Suturing
  • Joint injections
  • Skin biopsies
  • Basic wound care
  • Excellent communication, teamwork, and patient relationship skills
