JOB SUMMARY

The Nurse Practitioner is responsible for conducting a face-to-face encounter and documenting the clinical findings supporting a life expectancy of 6 months or less.

REQUIREMENTS

• Master’s degree in nursing from an accredited program with current license to practice professional nursing in the state in good standing
• Board-certified advanced nurse practitioner
• Current DEA
• CPR certification
• Minimum of 2 years of health care experience in hospital, medical outpatient clinic, home health, hospice, or specialty care setting
• Must be a licensed driver with an automobile that is in good working order and demonstrate ongoing insurance in accordance with state and or organization requirements
