Position Title: Licensed Social Worker – Home Health
Specialty: Social Services
Shift: Days, 5x8-Hour (08:00 - 16:30)
Job Duration: 13 weeks
Location: 1615 75th St SW Ste 210, Everett, WA 98203
Facility: Providence Hospice and Home Care of Snohomish County
Salary Range: Local-$55/hour or $2600 weekly
Job Duties:
- Provide skilled social work services in a home health setting, managing a caseload of up to 6 patients daily.
- Conduct assessments, develop care plans, coordinate resources, and deliver counseling to support patient and family needs.
Qualifications:
- LMSW license
- 1 year of skilled home health experience required
- BCLS certification
- COVID-19 vaccination and one booster required
