Position Title : Licensed Social Worker – Home Health

Shift : Days, 5x8-Hour (08 : 00 - 16 : 30)

Job Duration : 13 weeks

Location : 1615 75th St SW Ste 210, Everett, WA 98203

Facility : Providence Hospice and Home Care of Snohomish County

Salary Range : Local-$55 / hour or $2600 weekly

Job Duties :

• Provide skilled social work services in a home health setting, managing a caseload of up to 6 patients daily.
• Conduct assessments, develop care plans, coordinate resources, and deliver counseling to support patient and family needs.
