Ratio of Follow-Ups to New Evaluations : 10 : 1

Facilities & Schedule :

1.Kaiser Permanente Sandy Springs Medical Office - Sandy Springs, GA

2.Kaiser Permanente Southwood Comprehensive MC - Jonesboro, GA

Need is for one provider at Sandy Springs and one at Southwood.

4 days a week would be roughly 8-6 (10 hours)

5 days a week would be 8 : 30-5 (8 hours)
