Job Summary
Within the framework of the objectives and policies established by the Company, plans, coordinates and monitors home care services with clients and the office staff. Participates as a member of the company’s management team in planning, formulating policy, and supporting administrative decision making with an emphasis on care quality and caregiver preparedness. In summary, is responsible for client care, caregiver training, and oversight of the Quality Improvement Program.
Essential Functions
Maintains compliance with all care-related agency policies, procedures and regulatory requirements
Evaluates client status on a 60 day cycle, reviews and coordinates care plan with client needs
Reviews care staff for conformity with policies and procedures, care standards, state regulations and home care requirements
Evaluates the appropriate assignment of personnel to meet the specific need of individual clients and coordinates with office staff
Ensures the quality of patient care and service by using specialized knowledge and judgment
Performs care management activities for the individual clients, including but not limited to assessments, status evaluations, client, family and care staff teaching
Ensures that changes in the client’s medical or mental condition are reported to the client’s family, healthcare professionals and to Director of Operations.
Maintains visit records in accordance with Agency policy as well as state regulations as applicable
Assists in providing orientation and in-service education for Personal Care Assistants and Homemakers
Participates in quality improvement activities and evaluations of the Agency.
Investigates problems, irregularities, reported incidents, policy violations and other reportable events implementing the appropriate corrective action and follow-up.
May be required to travel on company business.
Participates in after-hours call rotation.
Additional (non-essential) Functions
Promotes Agency to the community, participating in community programs with other professionals when possible.
Other duties as assigned by the Director of Operations and Owners.
Education, Experience, Knowledge, Skills, Abilities and Availability
Graduate of a State approved school of vocational nursing with 6 months of satisfactory nursing experience
Currently licensed without encumbrances and in good standing as an LVN in the state of California.
Meets the health requirements for employees providing direct client care
Read, write, speak and understand English as needed for the job
Has a valid driver’s license
Working Conditions / Environment
Works both in office and in patients’ homes as required for assessments, client evaluations and monitoring quality of care
