We boast two primary care locations and 6 school-based health centers providing quality value-based care for over 20,000 patients. We provide services in Primary Care, Infectious Disease, Substance Use, Integrated Behavioral Health, Dental Services, Women's Health, and Pharmacy.

Position Summary :

We are currently seeking a Licensed Practical Nurse (LPN) for our Lincoln Heights Health Center location. This full-time role is essential in supporting clinical operations and delivering compassionate, patient-centered care.

Key Responsibilities :

• Assist providers with direct patient care and clinical procedures.
• Conduct patient interviews and accurately document medical information in the Electronic Health Record (EHR).
• Perform routine screenings and tests as ordered by the provider.
• Handle telephone inquiries, provide clinical advice within scope, and conduct follow-up as needed.
• Ensure exam rooms are properly stocked, sanitized, and equipped.
• Coordinate referrals and pre-authorizations, and communicate next steps to patients.
• Maintain patient confidentiality and comply with all HIPAA regulations.
