Location : Citrus County, FL and surrounding areas (including Ocala / Marion County)

Employment Type : Full-Time

Schedule : Monday–Friday

Compensation : Base salary up to $100,000 / year (DOE) + performance bonuses

Start : ASAP (urgent need)

About the Role

Bring high-quality primary care directly to patients where they live. In this community-based position, you’ll conduct home visits, manage chronic and acute conditions, and collaborate with a supportive care team to improve access and outcomes—while enjoying a consistent Monday–Friday schedule.

What You’ll Do

Conduct ~ 9 patient home visits per day across the assigned territory

Perform assessments, manage chronic / acute conditions, and prescribe per scope / DEA

Order and interpret labs / diagnostics; coordinate referrals as needed

Document encounters in eClinicalWorks (eCW) and close charts timely

Educate patients / caregivers on care plans, medications, and follow-up

Collaborate with physicians, care managers, and community resources

Requirements

Active Florida Nurse Practitioner (APRN) license

DEA certification

Valid driver’s license and reliable transportation (daily local travel)

Preferred

Experience in home-based primary care, community health, or house-calls

eClinicalWorks (eCW) proficiency

Strong communication skills and a patient-first mindset

Compensation & Benefits
