Responsibilities :

Provide direct patient care, including administering medications, wound care, and vital sign monitoring

Develop and implement individualized care plans in collaboration with the interdisciplinary team

Maintain accurate and thorough records of patient care and progress

Assist with resident transfers, repositioning, and mobility exercises

Collaborate with other healthcare professionals to ensure seamless care transitions

Participate in quality improvement initiatives to enhance the overall resident experience

Requirements :

Current LPN licensure in the state of Ohio

0 years of experience in a healthcare setting (new graduates welcome to apply)

Ability to work effectively in a fast-paced environment

Strong communication and interpersonal skills
