Job descriptionDirect Patient Care : Deliver hands-on nursing care, including administering medications (oral, injectable, inhalant, and intravenous), monitoring vital signs, and assisting with daily living activities. Care Coordination : Collaborate with other healthcare professionals to develop and implement individualized care plans tailored to each patient's needs. Patient Advocacy : Act as a primary caregiver and advocate for patients, ensuring their comfort and safety while promoting optimal health outcomes. Documentation : Maintain accurate records of patient care, including changes in health status, medication administration, and communication with family members and healthcare providers. Support for Chronic Conditions : Provide specialized care for patients with chronic illnesses, disabilities, or those recovering from surgery, ensuring they receive the necessary support for their conditions. Licensure : Must hold a valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in New Jersey State Education : associate or bachelor’s degree in nursing from an accredited institution. Experience : At least one year of home health experience required Clinical Expertise : Strong nursing skills and knowledge of medical procedures and protocols. Interpersonal Skills : Excellent communication and interpersonal skills to build rapport with patients and their families. Decision-Making : Ability to make sound clinical decisions independently, often without direct supervision. Compassion and Empathy : A caring attitude and the ability to provide emotional support to patients and their families.

Pay is $35 per hour LPN
$40 per hour RN
