As a Physical Therapist for Homeland HomeHealth, you would contribute to our community of caring by providing direct rehabilitation therapy to patients where they feel most comfortable, promoting and fostering their healing process.

We believe in a multiple-disciplined approach to help patients in their healing journey, which includes wound care, cardiac care, palliative care, fall prevention, rehabilitation services, intravenous therapy, and other care services.
As Homeland HomeHealth’s Physical Therapist, according to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Assistant Director of HomeHealth, the Physical Therapist (PT) evaluates and provides treatment to HomeHealth patients who are referred by the physician. The PT must have knowledge and understanding of a patient’s illness or injury and the ability to assess these needs and plan appropriate interventions. The role of the PT is to work as part of an interdisciplinary team to assist the patient in achieving his / her optimum goals
