About The Position

Job Title : Physical Therapist - Home Health

Experience : Minimum 1 full year Adult Home Health Experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have : Recent Home Health experience

Description :

Home visits for therapy services.

Periodic office meetings required.

Candidate must have reliable transportation.

Current driver's license and agency-required liability insurance are necessary.

Average number of patients per day is 5-6.

Contractor will be assigned between 5-10 patients per day.

Productivity points expectation is 30 per week.

Each type of visit and activity has a different point value.

Types of cases include Start of Care and Follow Ups.

Weekend requirements vary per location and profession.

Must be willing to travel up to 75 miles from the home clinic.

Mileage will be reimbursed at the IRS standard rate.

The commute to and from the first and last patient is not billable.

Local candidates are accepted at the same rate.

Pending licenses are not accepted; must have license in hand unless it takes a few days to obtain.

Typically, a standard schedule is Monday to Friday from 8 AM to 5 PM.

On-call and weekend rotation are not anticipated.

Expectations include accepting all patients scheduled.

Documentation must be completed within 24 hours of the appointment.

There are no guaranteed hours with this contract.

Clinician must meet weekly productivity points.

If not receiving enough patients, there may be opportunities to float to other clinics.

Experience with HCHB and Home Health software is needed.

All travelers must be proficient in relevant electronic medical records systems.
