Key Responsibilities :
Patient Care : Review and monitor patient medication regimens to ensure appropriate therapeutic outcomes. Work with the healthcare team to develop and adjust medication therapies.Consultation : Provide medication counseling to patients and healthcare providers, ensuring safe and effective use of medications.Medication Management : Oversee drug utilization reviews, formulary management, and medication safety practices across the institution.Education : Educate patients and healthcare teams on the proper use of medications, potential side effects, and new treatment protocols.Clinical Support : Assist in the development and implementation of clinical guidelines, protocols, and policies to improve patient care and safety.Qualifications :
Education : Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.Licensure : Active Illinois Pharmacist license in good standing (or eligibility for licensure).Experience : Clinical pharmacy experience preferred, particularly in hospital or outpatient settings.Skills : Strong communication and interpersonal skills, with a deep understanding of pharmacology, medication therapy management, and clinical practices.Why Illinois?
