You will promote the use of evidence-based practice through the nursing process, protocols, and procedures. Assess patient's status through interview, observation, and physical assessment of actual problems and outcomes. Develop an individualized plan of care using a collaborative approach with the patient and health care team. Use appropriate interventions identified in the plan of care to achieve expected patient outcomes. Systematically evaluate and document patients' progress toward expected outcomes on an ongoing basis throughout the shift. Modify the plan of care based on evaluation and as needed based on changes in patient condition. Complete required documentation as appropriate, including patient chart and event reports. Demonstrate professional accountability for your own practice by seeking resources and continuing self-development. Demonstrate technical knowledge of equipment and troubleshoot when problems arise. Acquire and maintain current knowledge and skills in evidence-based nursing practice. Promote the professional development of yourself and team members. Share clinical knowledge through informal teaching and in response to the learning needs of others. Demonstrate a practical problem-solving approach to patient moving and handling issues when applicable. Demonstrate skills to promote patient and staff safety through the appropriate use of lift equipment when applicable. Possess the knowledge and skills necessary to obtain adequate lab specimens for testing. Perform patient bedside lab testing and associated quality control in compliance with medical provider's orders and hospital lab procedures / policies. Coordinate the interdisciplinary care of patients through collaboration with team members and utilize additional resources when necessary. Analyze clinical situations systematically and act to obtain resources or correct problems to meet or exceed expected outcomes or patient needs. Recognize the need for assistance and utilize resources in a timely manner. Demonstrate ownership and persistence to ensure problems are resolved in the best way possible. Assume the assignment of non-complex and some complex patient management with some turnover in assignments. Consistently prioritize work based on changes in patient condition, changes in assignments, and unanticipated interruptions. Identify and utilize team resources to solve identified problems or needs. Collaborate with the health care team in the organization of tasks and activities for patient care from admission to discharge according to the plan of care. Work with other team members in delegating tasks and activities appropriately. Provide goal-focused shift reports. Advocate with physicians on patients' behalf when necessary. Utilize communication and teamwork strategies aimed at promoting high team performance and achieving optimal patient outcomes. Choose appropriate and meaningful communication styles when interacting with others and give and accept constructive feedback to / from peers. Take cultural, age-specific, and developmental factors into consideration when teaching patients regarding procedures, illnesses, and hospital experiences. Ensure patient safety in the hospital environment by following policy and procedures. Consult appropriate resources within the hospital to provide specialized education. Utilize patient education systems as a resource for patients and family members. Actively participate in data collection, analysis, and problem-solving initiatives. Ensure that care provided is consistent with standards of care and evidence-based practices. Work with the interdisciplinary team to identify opportunities for improvement in patient care and satisfaction. Promote and achieve customer satisfaction among patients, families, departments, and physicians. Participate in the implementation of performance improvement initiatives. Follow up on patient / family requests and anticipate their needs. Encourage coworkers to achieve excellent customer satisfaction using organizational service excellence standards. Serve on unit-based councils and identify practice problems. Implement changes in clinical practice based on literature and unit improvement outcomes.
