This role serves individuals with learning, developmental, and behavioral health disabilities through direct support and person-centered advocacy. Case Management: Assess client needs on an ongoing basis to identify service gaps and coordinate care accordingly.
Life Skills Coaching: Teach money management, budgeting, and banking skills to promote financial mastery.
Advocacy & Liaison: Act as the lead contact for families, employers, and external agencies such as the NJ Division of Developmental Disabilities.
Health & Safety: Conduct monthly home visits to ensure safe, hygienic living environments and manage calendars for vital medical appointments.
Documentation: Maintain accurate electronic health records (EHR) and clinical progress notes in a timely, legible manner.
Education: Bachelor’s degree in Social Work, Psychology, Sociology, or Human Resources.
Experience: 3 years of related experience with a degree, or 10 years of consistent experience in the field.
Licensure: A valid New Jersey driver’s license is required.
Certifications: Must hold a valid CPR/First Aid certification.
Technical Skills: Familiarity with EHR systems (Carelogic preferred), Microsoft Word, and Excel.
Compliance: Successful clearance of a NJ background check and drug screening.
Regularly required to talk, hear, stand, and walk.
Must be able to use hands to finger, handle, or feel, and occasionally lift up to 10 pounds.
Compensation / Pay Rate (Up to): $50,000.00 - $55,000.00 Per Year
