We’re thrilled to offer a $5,000 sign-on bonus and a relocation package of $5,000 (for moves over 100 miles) or $2,500 (for moves over 50 miles) for experienced nurses joining our team!

Position Overview : This role will primarily serve the Carle Health Watseka Clinic and is a hybrid position with 1 day remote and 4 days in the office.

Your Role

As an Outpatient Care Management RN , you will play a vital role in providing care management and population health services to our patients. Your focus will be on :

  • Coordinating care for patients with chronic diseases.
  • Facilitating medication management.
  • Managing care transitions across various settings.
  • Creating personalized care plans tailored to each patient.
  • Building effective partnerships with patients, families, and healthcare teams.

Key Responsibilities

  • Communicate and coordinate care across the healthcare continuum.
  • Identify and connect patients with appropriate providers and community services.
  • Educate patients on medication adherence and self-care to reduce avoidable readmissions.
  • Conduct holistic health assessments, considering health risks, preferences, and social determinants.
  • Ensure accurate documentation in patient Electronic Medical Records (EMR).
  • Utilize data and reporting mechanisms to evaluate performance and outcomes.

    • Qualifications

  • License / Certifications :

    • Licensed Registered Professional Nurse (RN) in Illinois.

  • Certified Case Manager within 2 years (CCMC).
  • Education : College Diploma in Nursing (Required).
  • Experience : 3+ years in a relevant field.

    Compensation and Benefits

    The compensation range for this position is $30.39 - $52.27 per hour . Actual compensation will depend on various factors, including experience and qualifications.
