The Progress Center is currently seeking full-time Behavioral Health Home Children's Case Manager's in our Oxford location! As a Behavioral Health Home Children's Case Manager, you will assist families and children experiencing intellectual and / or developmental disabilities. This role is heavily centered on connecting families with resources within their communities that help support and foster greater independence. BHH Services aim to identify the medical, social, educational and other needs, identify the services necessary to meet those needs, and facilitate planning in accessing those services. BHH consists of intake / assessment, plan of care development, coordination / advocacy, monitoring, and evaluation.
- This position covers several counties with a primary work location at our Oxford office. Some remote work is possible and regular travel is required.

Complete assessment and / or coordination of assessment of the consumer's medical, social, educational, and other needsDevelop and implement a plan of care under the direction of the consumer, his / her guardian and other team membersCoordinate service providers and resources as described in the plan of careLink consumer to appropriate agencies, community resources and informal support systemsMonitor the consumer's progress towards achievement of the objectives identified in the person centered plan, including evaluation of the plan itself on a regular basisParticipate in staff meetingsPerform other duties and responsibilities as pursuant to the policies and procedures manual and the supervisor.Update and maintain client information on electronic records.Requirements :
Bachelor's degree with specialization in psychology, behavioral health, social work, special education, counseling, rehabilitation, nursing or related fieldTwo years' experience in working with disabilitiesProficiency with electronic software (Microsoft Office Suite) and electronic health records. Strong writing, typing and organizational skills are necessary. Strong interpersonal skills, and a professional presentation are required.Comply with all federal, state and local regulations as regards services for persons with developmental or intellectual disabilities or autism, including rights of recipients and mandatory reporting.Comply with Office of Aging and Disability Services and Department of Human Services regulations.Demonstrate respect and add value to the lives of those we supportFulfill all training requirementsParticipate as a member of the Team and join a tight knit group of professionalsEducation :
