full-time Evening Hospitalist (Internal Medicine) to join its

inpatient care, manage admissions, and support continuity of care

in collaboration with the daytime and overnight teams. Lead care

coordination with interdisciplinary teams – nursing staff,

consultants, case managers, and ancillary services – for optimal

outcomes, and help implement process improvements in inpatient

medicine. Serve as a professional leader and role model,

participating in hospital committees, promoting best practices, and

ensuring patient safety and satisfaction. with completion of an

ACGME – accredited Internal Medicine Residency. Board-Eligible or

Board-Certified in Internal Medicine. Must secure a Connecticut

medical license, Controlled Substance Registration, and DEA

certification. Must obtain attending medical staff privileges at

respective YNHHS entity. Two (2) years caring for medicine

inpatients as resident and / or attending physician. Hold a valid
