Schedule : Part Time or Full Time - 40-80 hours / pay period (You Pick!) - Monday to Friday - 8 a.m. to 4 : 30 p.m.

As a Physical Therapist, you'll be at the heart of our community, providing hands-on therapeutic support to clients while fostering meaningful connections. Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you'll play a key role in helping clients achieve their highest level of independence and well-being.

As a Physical Therapist, you will :

• Deliver Direct Client Care - Conduct comprehensive evaluations and develop personalized treatment plans with short-term and long-term goals.
• Maintain Accurate Client Records - Document services accurately and timely using Vivie's EMR system, ensuring compliance with all payer sources and authorization requirements.
• Facilitate Follow-Up Programs - Recommend and oversee continuation programs to support long-term client success and optimize discharge outcomes.
• Collaborate and Communicate Effectively - Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
• Ensure Regulatory Compliance - Complete and document supervisory visits with PTAs and RA / CNAs in accordance with state and regulatory guidelines.
