Assignment Details / Requirements :

Board Certified or Board Eligible within 5 years of residency

Indiana license at the time of submission required

Indiana DEA and CDS certification is required

ACLS certification required

Clinic Coverage (7AM-5PM); Night Call (5PM-7AM)

Adults only

Average 2-5 patients per day

EMR : EPIC
